Diagram 2 and 3 represent the process of activation of a neural network using the gate analogy.
Note that neural plasticity can work both ways to promote or degrade neural networks based on biopsychosocial influences. Specifically, Kalat (2013) discusses “the Concept of the Synapses” wherefore the neurons receive external stimulus (gate openers or closers) from the axions and subsequently releases various chemicals known as neural transmitters (neuropeptides, hormones, amino acids…or an array of over 100 transmitters depending on the stimulus) that are exchanged between neurons by synapses. In short, Kalat explains that these transmitters depolarize or excite “action potential” allowing chemical exchanges to occur between neurons. The process called reuptake occurs and transports proteins that can impact genes.
The attached diagram displays some of the socio-environmental factors that influence chronic pain based on this process. The mind and body are truly one, and multi-modal or integrative treatments are the “new way forward” to address chronic pain.
Take-Aways!
1. CBT therapy for pain can close the bad gates and open new ones through new learning and growth (Neurogenesis).
2. Chronic pain is influenced by a number of mind body factors.
